Dental Implants can sometimes be placed without surgical incisions. In this technique, the dental implant is placed directly through the gums and into the underlying bone. This method is called "flapless surgery". Traditionally, an incision is made in the gums and the gum tissue is "flapped" away from the bone. The advantage to the "flapless" method is there is minimal trauma to the soft tissues because no incisions are made. The gums and bone are penetrated with a drill only. The recovery for the patient is faster with less discomfort. The disadvantage is that the surgeon does not get to view the underlying bone. The underlying bone usually has a totally different shape and contour than the overlying soft gum tissue. With modern conebeam 3-D scanners the shape of the bone is known without opening the gum tissues. However, often the gum tissues are opened to modify the bone or tissue. A dental implant replaces missing teeth by replacing the missing tooth root.Dental crowns or bridgework are then built on the implant foundation.
Dr. Paul Kozy, a Toledo, Ohio implant dentist states, " We are doing more and more flapless implant placement, however, the surgeon must have considerable experience in "open" surgery to appreciate where the underlying bone is located even with modern 3-D scanners.". Dr. Kozy further emphasizes, " We are seeing dentists promote flapless surgery and mini implants as if these techniques are better for the patient. In reality, flap surgery is usually necessary to make the bone and gum tissue in the proper shape to best support the dental implants. Flapless implant placement should only be done in select situations by experienced implant dentists."
