Kym Hance, B.S.W., CMC is the new Director of Client and Community Services. Hance will oversee all Case Management programs including the Nursing Home Transition Diversion Waiver program. Hance received her B.S.W. from Barton College in Wilson, NC. She also holds CMC (Care Manager Certified) professional certification. She resides in Middle Grove, NY.

Pat Hughes, R.N., M.B.A., is the new Director of Clinical Services. Hughes will oversee all clinical areas of the home health agency, including working closely with the caregivers and home aides to assess, coordinate and assure quality of care for all home-care clients. Hughes received her R.N. degree from St. Joseph's Hospital Health Center in Syracuse, NY and her M.B.A. from Rivier College in New Hampshire. She also holds certification in Quality and Risk Management. She resides in Saratoga Springs, NY.

Ann St. Croix, B.S., M.S., is the new Director of Operations. St. Croix will oversee all staff and provide outreach to and recruitment of clients throughout Saratoga, Schenectady, Warren, Washington and Upper Albany counties. She will be responsible for hiring and retaining staff, including overseeing pre-employment applications, background checks and verifications of licenses and certificates. St. Croix received her B.S. degree in Applied Science Sociology Management from the State University of New York at Binghamton and her M.S. degree in Gerontology, Long Term Care Administrative Track from North Texas State University?s Center for Studies in Aging. She resides in Ballston Lake,
NY.
