Pedagogy, Inc. emphasizes Long Term Care facility strategies to reduce re-hospitalizations

Implementing a strategy to significantly reduce hospitalizations is extremely important for the Long Term Care facility to improve patient care and satisfaction, profitability, and even survival.

The push to reduce transfers to acute care facilities in the Long Term Care industry, both from a patient care and financial perspective, is the basis for this announcement.
There appears to be little doubt that LTC facilities will face penalties for not reducing hospitalizations. The current administration's recommendation for significant cuts in reimbursement rates for SNF's with excessive hospital admission rates virtually mandates that facility management evaluate policies and procedures which affect re-hospitalization, implement necessary improvements, and educate their staff in order to survive. Facility nurses must have the support of management, medical directors, nurse practitioners, and on-call doctors to truly engage in a program to reduce hospitalizations.
Many LTC facilities have learned that they can utilize the implementation of such a program as a successful marketing tool.
