A voice modem with TAPI drivers supported by Windows
A sound card supported by Windows
A telephone line splitter to enable your modem and phone to be connected to a telephone jack in parallel.

Voice modem

A microphone and speakers connected to your sound card if you wish to record and playback voice through the sound card
Foot pedals connected to a serial port on your computer.

For automatic conversion of recordings to MP3 format, a suitable MP3 encoder that provides a command line interface.
